The NMC LOTAG18 lockout tag is made of 15 mil vinyl for water and chemical resistance, and has a red, white, and black "Danger" logo, "Equipment Lock-Out," and diagonal red and white stripes on both sides for high visibility. Text on the front of the tag reads, "Equipment Lock-Out - A Life is On The Line!" followed in smaller print by "This tag & lock to be removed only by the person shown on back." Text on the back of the tag reads, " Equipment locked out by" followed by four blank lined areas labeled "Name," "Dept.," "Date," and "Time." This tag has a tensile strength of 6,300 pounds per square inch to help resist unauthorized removal. It includes a 3/8" diameter hole for fastening and a reinforced brass grommet for longer wear. This tag is part of a lockout/tagout (LOTO) safety procedure required at facilities where a device or energy source needs to be isolated during maintenance and repair.
